Advance Medical Directives (AMD)

What is an Advance Medical Directive?
An Advance Medical Directive (AMD) is a medical option that allows an adult to choose, in advance, their medical wishes towards the end stages of their life. This document only applies to the following situations:
- Terminal illness (e.g.: the final stages of cancer).
- Persistent vegetative state, or a state of non-reversible coma.
- Other end-stage and non-reversible illnesses.
Through an AMD, you can clearly choose to accept or reject particular life-sustaining treatments, such as CPR. This allows medical staff and family members to understand and honour your choices. The AMD does not apply to minor illnesses.
According to the Advance Decision on Life-sustaining Treatment Ordinance, any adult who is of clear mind can use the standard form to create an AMD. It is also recommended to discuss your wishes with your family and medical providers, to ensure that your wishes will be carried out.
Contents to be included in an AMD

Refusal of life-sustaining treatment
1. Invasive treatments such as tracheostomy
2. Emergency life-sustaining treatment, e.g. CPR

Providing and offering
1. any general care that is essential to maintaining the comfort of the patient, or palliative care, e.g. pain relief and warmth etc.
2. food and drink to the patient for the patient’s consumption by mouth

Advantages of having an AMD
- To act in the best interests of the patient, to relieve their suffering, to give them the right to choose not to receive invasive treatments and to retain their dignity.
- When the doctor asks the family whether to continue treatment, a lack of consensus may lead to disputes among family members. It is always difficult to make such decisions, so having an AMD makes it easier for relatives and healthcare professionals to follow and avoid disputes.

Maker
A competent person over 18 years of age, who refuses certain life-sustaining treatments when he/she is incapable of self-determination at the end stage of his or her life

Witnesses
Must not be a beneficiary and one of them has to be a registered medical practitioner
